Teens are given opportunities to explore their gifts and talents through leadership, discipleship, and missions while focusing on becoming fully devoted followers of Jesus.

Medical Permission Form
The Annual Permission form is needed for any and all events at Christ Church. This form needs to be signed by the parent's or guardian of the teen who is participating with Christ Church student ministries.
Out of Area Medical Form
Due to the new HIPPA regulations, this form must be completed for any trip more than 2 hours away.
Superfund Application
Want to participate in a trip or event, but you're low on cash? Fill out the Superfund Application and be "hired" by people in the church to do different jobs. Money you earn will be put into your account and will be used for trips, retreats, and events.
